POSITION OVERVIEW

• Works in collaboration with the Medical Director to develop policies and procedures and protocols for medical services.
• Acts as the Most Responsible Provider/Prescriber for residents of the hospice and works within her/his legislative scope of practice as described and outlined by the College of Nurses of Ontario (CNO).
• Collaborates with the resident and family/caregiver, and the health care team in the development, implementation, and evaluation of the resident’s plan of care; provides leadership and mentorship to staff to enhance their knowledge, assessment skills, and ability to care for residents at end-of-life; and leads and collaborates in research, education, and evidence-based practice initiatives to optimize the resident’s experience.

DIRECT CARE TO RESIDENTS

• Provides comprehensive palliative care to residents as part of the interdisciplinary care team.
• Increases continuity of care through collaboration, consultation, and referral as appropriate:
• Collaborates and consults with physicians, nursing staff, interdisciplinary team members and external resources regarding the achievement of the residents’ goals of care.
• Resident includes substitute decision makers as appropriate. Interdisciplinary health care team includes unregulated health care providers and hospice volunteers.
• Assists, supports, guides and provides consultation to the nursing staff and interdisciplinary team members regarding challenging clinical situations.
• Makes referrals to specialized consultants, services, and other health providers as necessary.
• Accepts referrals from inter-agency team members.
• Performs a person-centered health assessment of residents on admission, and as needed (including weekly medication reviews). Conducts person-centered comprehensive health histories.
• Engages with residents/SDM in regular dialogue about their goals of care. Participates in care conferences.
• Orders and/or performs appropriate screening and diagnostic investigations, interpreting results and assuming responsibility for follow-up.
• Manages the care of residents by providing pharmacological, complementary and/or counseling interventions, and performs procedures within the NP scope of practice.
• Documents clinical data, assessment findings, diagnoses, plans of care, therapeutic interventions, resident responses and clinical rationale in a timely and accurate manner and reports appropriately to interdisciplinary team members.
• Participates in providing after hours and on-call coverage in accordance with the on-call policy of the Hospice.

TEACHING AND COACHING

• Provides formal and informal teaching and coaching by integrating best practices in the management of clinical care to interdisciplinary team members, serving as resource person, educator and role model and contributes to the performance appraisals of nursing, PSW and axillary staff.
• Provides clinical placements and precepts NP students interested in specializing in palliative care or who are enrolled in a Post-Masters Certificate in Palliative Care.
• Participates in identifying, analyzing, and interpreting trends in resident care outcomes and professional nursing practice issues to determine priorities for educational programming.
• Contributes to planning, implementing, and evaluating learning resources and health education programs for residents, families, and substitute decision makers.

RESEARCH

• Engages in evidence-informed practice by critically appraising and applying relevant research, and theory in providing health-care services.
• Collaborates with members of the interdisciplinary team and/or community to identify research opportunities and to conduct and/or support research.
• Acts as a change agent through knowledge translation and dissemination of new knowledge that may include formal presentations, publications, informal discussions, the development of best practices, policies and procedures.

QUALIFICATIONS

Education (Degree/Diploma/Certifications)

• Degree/Diploma in Health Care Administration or related field.
• Current registration with the CNO as a Registered Nurse in the Extended Class (Primary Health Care or Adult) and licensed to prescribe Controlled Substances and Narcotics.

Experience

• In-depth understanding and minimum 5-years-experience in Hospice Palliative Care.
• Experience in not-for- profit sector.
• Significant management experience including leadership, supervision of staff and volunteers, program development/ evaluation, strategic planning, goal setting, records management, finance development, budget preparation, community development.

Knowledge, Skills and Abilities

• Consultative team-based management style and strong problem solving skills.
• Effective verbal and written communication skills.
• Ability to work independently, to exercise sound judgment, to make appropriate decisions and to accept responsibility for outcomes.
• Positive approach to client service and a demonstrated capacity for empathy.
• Commitment to teamwork and ability to collaborate effectively.

General

• The ability to work flexible hours to accommodate meetings and to be accessible to staff and clients.
• Strong Computer skills in Word, Excel, Access, and PowerPoint.
• Valid Driver’s License, use of reliable vehicle and willingness to travel.
• Provide current Vulnerable Sector Police Record Check.
• Is fully vaccinated against/immune to Tuberculosis (two-step Tuberculin testing only), Measles, Mumps, Rubella, Varicella (chickenpox), Hepatitis B, Tetanus/Diphtheria/Pertussis (whooping cough), Influenza, and COVID-19.
